Community Support and Community Support Team Services
This service is provided in the homes, schools, detention centers, jails, (state funded only), homeless shelters, etc. Services include, but not limited to: Therapeutic Mentoring, Socialization and Adaptation Skills, Behavior and Anger Management, Daily and Community Living, Vocational and Educational Counseling.
Case Management
This service includes, but is not limited to: Development and Coordination of the Person-Centered Plan (PCP), Linking and Arranging for Services, Providing Resources and Referrals, Parent/Caregiver Training, Substance Abuse Education, 24-hour Crisis Response Line.
Clinical Assessment
This service is an intensive clinical and functional face-to-face evaluation of a consumer's mental health or substance abuse condition that results in the issuance of a comprehensive report, with regards to services, and provide the basis for the development of a Person-Centered Plan. The assessment will also evaluate the consumer's level of readiness and motivation to engage in treatment and to meet clinical needs.
Intensive In-Home Services
A time limited (3-6 months) program designed to provide youth and families with assistance and is delivered primarily in the family's home and requires participation from the entire family in order to achieve the most favorable results. Initially, the services are more intense but contacts are reduced as progress is made. Our teams are available for crisis response 24/7/365. Services include, but are not limited to: Diffusing Current Crisis, Prevent Out-Of-Home Placement, Improving Parenting Skills, Coping Skills, Problem-Solving Techniques.
